This account has not been discharged in a bankruptcy.

I am not an active member of the United States Military or its Allies.

Recurring Payments: By agreeing to these terms and conditions, I authorize Stenger & Stenger, P.C. to process recurring payments for any plan that I setup. Such payments shall be made via Electronic Funds Transfer (EFT).  I am responsible for ensuring that my chosen payment method has sufficient funds or credit available to cover each recurring payment. Failure to maintain a valid payment method may result in suspension or termination of my service. I agree to update my payment information promptly if there are any changes to my preferred payment method.
